Essential components of each evaluation of appropriateness begin with a formal referral by an actively licensed medical professional with whom you currently have a professional relationship. After any release(s) of information have been signed, ongoing professional conversations between Tammy and that professional will be an essential component throughout the period of services. Other service-related forms specific to the services you seek will be provided after receipt of and agreements regarding the initial referral.
