Essential components of each evaluation of appropriateness begin with a formal referral by an actively licensed medical professional with whom you currently have a professional relationship. Once you have signed a form allowing the release of information, ongoing professional conversations between Tammy and that professional will be an essential component throughout the period of services. Other forms in this regard related to the services you seek will be provided after receipt of that formal referral as well as agreements regarding the initial referral.
